Sontag wrote Illness as Metaphor in 1978, while suffering from breast cancer herself. In her study she reveals that the metaphors and myths surrounding certain illnesses, especially cancer, add greatly to the suffering of the patients and often inhibit them from seeking proper treatment. By demystifying the fantasies surrounding cancer, Sontag shows cancer for what it is - a disease; not a curse, not a punishment, certainly not an embarrassment, and highly curable, if good treatment is found early enough.

In 2001 she was awarded the Jerusalem Prize for the body of her work, and in 2003 she received the Prince of Asturias Prize for Literature and the Peace Prize of the German Book Trade. 问题在于就算我们知道了疾病被作为隐喻的逻辑我们就能仅仅把疾病当成疾病吗?前面提到肺结核和癌症的隐喻生产过程中它们的神秘感起到了重要作用,但是对于一些疾病比如说艾滋,对它们的科学认知(知道艾滋通过性传播且肛交是高危性行为)并没有消除或减弱隐喻,而艾滋似乎是更加稳定地与同性恋绑定在了一起。
